CareCollab specializes in coordinating comprehensive care for vulnerable populations struggling with chronic medical conditions, mental health diagnoses, substance-use disorders, and homelessness.

At CareCollab, our mission is to provide integrated and thorough care for our members within the home and community, aiming to reduce hospitalizations, recidivism, and care gaps. We ensure holistic support for our members through centralized care plans and dedicated oversight. Our commitment is to promote total well-being, guiding individuals toward lasting health and resilience.

CareCollab is a free and voluntary care management service for Medicaid recipients.

Each eligible member is assigned a dedicated Care Manager who works with their existing care team – such as doctors, therapists, social workers, and social service providers – to advocate for the member and their needs.

Comprehensive care management

The creation of a personalized care plan allows our care managers to oversee the entire care process and ensure the member’s wellness is being prioritized and cared for at every stage.

Care, coordination, and health promotion

Our care managers serve as healthcare advocates in every part of the member’s personalized plan of care in collaboration with their care team

Use of health information technology (HRT) to link services

Utilization of HIT to facilitate communication between health care providers, improve medication adherence and to ensure that a high-level of care is received from all providers

Comprehensive transitional care

Our dedicated team assists with coordinating the transfer of care between care settings (hospitals, rehab, home) and follows up on adherence to the post-discharge plan

Member and family support

Our care managers prioritize involving those invested in the member’s wellness in every aspect of the care plan, ensuring comprehensive support for both the member and their caregivers.

Referral to community and social support

We aim to connect our members to the community services and social service programs they are eligible for and would benefit from.

What is CareCollab?

CareCollab is a dynamic care management company that provides free, voluntary care management services for Medicaid recipients in New York City, Westchester County, and Nassau County, working to integrate physical health, mental health, and supportive social services. 

Who is eligible for CareCollab's services?

Eligibility requires that you have active Medicaid, are aged 21 or older, live in NYC, Nassau, or Westchester County, and have at least one qualifying condition such as two or more chronic conditions (medical, mental health, or substance misuse), HIV/AIDS, serious mental illness, or sickle cell anemia. 

Is there a cost for CareCollab's services?

CareCollab's services are completely free for all eligible Medicaid recipients as part of our commitment to improving community health outcomes.

How does the care management process work?

The care management process begins with assigning each eligible member a dedicated Care Manager who collaborates with their existing care team of doctors, therapists, and social workers to coordinate care, advocate for their needs, and connect them to essential services within their community. 

Will CareCollab replace my current healthcare providers?

CareCollab works as a complement to your existing healthcare providers, enhancing coordination and support while maintaining your current medical relationships and care team structure.

How do I make a referral to CareCollab?
